RUMBALARA HOME AND COMMUNITY CARE (HACC)
Rumbalara HACC Program provides home and community support services for frail Elderly people over 45 years of age, younger people with disabilities and their carers.
These services aim to provide support for people living at home for as long as possible and to avoid needing to go into residential care.
ELIGIBILTY
An assessment of the needs of people who wish to become HACC clients, or have been referred to the HACC service by someone else, is the first step in deciding whether you are eligible for HACC Services, and if eligible, the type of services they need.
The assessment is done by a Rumbalara HACC assessment officer.
They make a time to see the person wanting HACC services and the assessment is usually conducted at the client’s home and may require more than one visit.
In Indigenous communities, It is important that assessments are done in a way that is culturally appropriate.
